LiceSisters is offering Tamarack campers two treatment options:

Option 1:

$395

2 treatments and 2 comb-outs 7-10 days apart

Option 2:

$150

1 treatment, buzz cut, and a follow-up head check 7-10 days later

Consent to Treatment

I hereby request and consent to Lice Sisters’ inspection, assessment and treatment of me, and all members of my family (including but not limited to my minor child or children) (the “Clients”). Anticipated services include the inspection of Clients’ hair and scalp, assessment of appropriate courses of treatment, and lice treatment services (including but not limited to the removal of nits and/or lice from Clients’ hair, and treatment with Lice Sisters’ organic proprietary lice removal product (“Lice Sisters Product”).)

Guarantee

Lice Sisters has developed protocols for the removal of nits and/or lice, and the treatment of lice. Provided Clients strictly follow Lice Sisters’ protocol and directives, Lice Sisters offers a limited guarantee of its treatment. Specifically, provided Lice Sisters’ protocol and directives are strictly followed, Lice Sisters agrees to re-inspect, re-assess and re-treat clients for up to two weeks from the date of Clients’ most recent treatment. Refunds will not be issued for any payments made to Lice Sisters.

Risks

While Lice Sisters is dedicated to the safe, effective and efficient eradication of lice, there are health risks and complications in any condition and any form of treatment. These risks include but are not limited to discomfort, irritation, adverse reaction to or illness in response to treatment and/or the application of Lice Sisters’ Product. Clients acknowledge being informed of all risks, and notwithstanding such risks, give informed consent to treatment.

Payment

Clients shall pay Lice Sisters pursuant to its then current schedule of costs and charges. Payment is due the sooner of Lice Sisters’ initial treatment or demand for payment. Clients understand and agree that Lice Sisters’ schedule of costs and charges may be changed from time to time.

Disclaimers

Lice Sisters and its staff are not licensed physicians. Clients, therefore, affirm that they do not expect Lice Sisters to meet, nor will they attempt to hold Lice Sisters to, any medical professional standard of care. Lice Sisters and its staff disclaim any and all responsibility for damage to Clients, third persons or property arising from Lice Sisters’ lice treatment services, Lice Sisters’ Product, or motor vehicles or equipment used by Lice Sisters and its staff.

Course of Treatment

This consent extends to all courses of treatment for Clients’ present and any future condition(s).

Agreement to Arbitrate

It is Lice Sisters’ goal to fully satisfy Clients. In the event Clients are dissatisfied in any way by Lice Sisters’ inspection, assessment or treatment, or Lice Sisters’ Product, Clients, as well as Clients’ personal representatives, trustees, heirs and present or future spouses agree to arbitrate any dispute, controversy or claim, including claims for physical injury or wrongful death, emotional distress and for loss of consortium, before a neutral arbitrator(s) in Southfield, Michigan pursuant to the rules of the American Arbitration Association, and a court of competent jurisdiction shall confirm and enter judgment upon any arbitration award, or, if appropriate, vacate or modify any arbitration award.
